ANALISA PENYEBAB TERBAKARNYA HEAVY DUTY DUMP TRUCK 785-7 DI PT. UNITED TRACTORS SITE DAMAI

Heavy duty (HD) 785-7 is one of four units of heavy equipment that is type dump truck, HD 785-7 commonly used in mining industry, used to transport materials like the sand, gravel or ground for purposes of industry. It was often in operate it mining industry therefore very important to renovate in accordance with procedures in units of heavy equipment. a few fault in doing maintenance over four units of heavy equipment and can be fatal in units and of the unit operator as in PT. United Tractors site Damai, which one HD 785-7 who was there was fire accident when,the unit was burning in the upper right front of the unit. Working in the field the fire was caused by accident in units of the fault to hose maintenance in the unit. Hence writers are interested in the research unit in the accident, fire to find the caused of the fire accident where the fire accident because a fault in the maintenance and do not apply the kawashima method for hose structuring and positioning on one HD 785-7. Heavy Duty (HD) 785-7 adalah salah satu unit alat berat yang merupakan tipe Dump Truck, HD 785-7 biasa digunakan dalam industri pertambangan, digunakan untuk mengangkut bahan material seperti pasir, kerikil atau tanah untuk keperluan industri. Dump Truck tipe HD785-7 melakukan pekerjaan dimana isi muatannya di isikan oleh alat pemuat seperti excavator, sedangkan untuk membongkar muatannya dump truck tipe ini mengangkat bagian vessel dengan menggunakan tenaga hidrolik dengan cara di tumpahkan ke belakang. Karena sering di operasikan di industri pertambangan maka dari itu sangat penting untuk melakukan perawatan sesuai prosedur pada unit alat berat. Kesalahan sedikit saja dalam melakukan perawatan terhadap unit alat berat maka dapat berakibat fatal pada unit maupun pada operator unit tersebut. Seperti yang terjadi di PT. United Tractors site Damai, dimana salah satu HD 785-7 yang berada disana mengalami fire accident pada saat sedang beroperasi di lapangan, unit terbakar pada bagian depan kanan atas unit. Terjadinya fire accident pada unit tersebut disebabkan karena kesalahan dalam melakukan perawatan hose pada unit tersebut. Oleh karena itu penulis tertarik melakukan penelitian terhadap unit yang mengalami fire accident tersebut, dengan mencari faktor-faktor penyebab utama terjadinya fire accident dimana terjadinya fire accident dikarena kesalahan dalam melakukan perawatan dan tidak menerapkan metode kawashima untuk penataan dan pemosisian hose pada salah satu HD 785-7.
